Sacramental Certificate Request

To request a Sacramental Certificate from Mary, Mother of the Church, please contact the Parish Office.

In order to protect confidentiality, certificates can only be requested by the individual named on the certificates, the parent or guardian of a child (under 18), or a parish or diocese. Adults (18+) must request their own certificates.
